Objective: The aim of this study was to describe the epidemiology of childhood cancer in Chile and the disease landscape, assessing achievements, collaborations, and future challenges to be addressed by the National Plan for Child and Adolescent Cancer Control. Methods: This descriptive study provides a general overview of national and international collaboration strategies and discusses the results of the Third Childhood Cancer Surveillance Report (2017-2019), the St. Jude Pediatric Oncology Facility Integrated Local Evaluation Tool (or PrOFILE) report, collaboration with the Pan American Health Organization within the framework of the Global Initiative for Childhood Cancer and the development of the National Plan for Child and Adolescent Cancer Control within the CureAll framework. Results: The analysis reveals the impact of childhood cancer by considering the incidence between 2017 and 2019, encompassing gender disparities. Leukemia is the most frequently occurring type of cancer, accounting for 40.4% of cancers among children and adolescents younger than 15 years and with an incidence of 57.5 cases per 1 million children in this age group. Cancer is the second leading cause of death among those aged 5-14 years. Cancer survival increased between 2007 and 2019, with 78.4% survival at 5 years post-diagnosis in 2023. The development of the National Plan for Child and Adolescent Cancer Control involved assessing the situation, setting goals and devising an action plan to reduce mortality from cancer in childhood and increase survival rates through early interventions and smooth transitions to adult care. Conclusions: Chile has made progress in childhood cancer indicators, particularly in increasing survival, demonstrating its commitment to improving care for children with cancer, and this has been achieved through legislative frameworks, national planning, collaborative partnerships and participation in global initiatives. Despite the progress made, ongoing research, strong policy implementation and multidisciplinary collaborations remain vital to addressing persistent challenges. This study highlights the need to refine health systems, data collection methodologies and global cooperation to ensure optimal care for every child facing cancer, thus improving their chances of survival and their overall quality of life.

El objetivo de este estudio fue describir la epidemiología del cáncer infantil en Chile y el panorama de la enfermedad mediante la evaluación de los logros, colaboraciones y desafíos futuros a abordar por el Plan Nacional de Cáncer Infanto-Adolescente. Métodos. En este estudio descriptivo se proporciona una visión general de las estrategias de colaboración nacional e internacional y se analizan los resultados del Tercer Informe de Vigilancia del Cáncer Infantil (2017- 2019), el informe de la herramienta de evaluación local integrada del Centro de Oncología Pediátrica del St. Jude Hospital (PrOFILE, por su abreviación en inglés), la colaboración con la Organización Panamericana de la Salud en el marco de la Iniciativa Mundial contra el Cáncer Infantil y la elaboración del Plan Nacional de Cáncer Infanto-Adolescente en el marco CureAll. Resultados. En el análisis se evidencia el impacto del cáncer infantil si se considera su incidencia entre el 2017 y el 2019, incluidas las disparidades según el género. La leucemia es el tipo de cáncer más frecuente, representa el 40,4% de los cánceres en la población infantil y adolescente menor de 15 años y tiene una incidencia de 57,5 casos por millón en este grupo etario. El cáncer es la segunda causa de muerte en la población de 5 a 14 años. Entre el 2007 y el 2019 se produjo un aumento de la supervivencia de los pacientes con cáncer, que en el 2023 alcanzó el 78,4% a los 5 años del diagnóstico. La elaboración del Plan Nacional de Cáncer Infanto-Adolescente implicó evaluar la situación, fijar metas y diseñar un plan de acción para reducir la mortalidad por cáncer en la infancia y aumentar las tasas de supervivencia mediante intervenciones tem- pranas y transiciones graduales al cuidado en la edad adulta. Conclusiones. Chile ha logrado avances en los indicadores del cáncer infantil, en particular en cuanto al aumento de la supervivencia, que reflejan su compromiso en mejorar la atención prestada a la población infantil con cáncer. Esto se consiguió mediante marcos legislativos, planificación nacional, asociaciones de colaboración y participación en iniciativas mundiales. A pesar de los avances realizados, las investigaciones en curso, una sólida aplicación de las políticas y la colaboración multidisciplinaria siguen siendo cruciales para abordar los desafíos que subsisten. Este estudio subraya la necesidad de perfeccionar los sistemas de salud, las metodologías de recopilación de datos y la cooperación mundial a fin de garantizar una atención óptima para la totalidad de la población infantil que afronta un cáncer, con la consiguiente mejora de su prob- abilidad de supervivencia y su calidad de vida en general.


[RESUMO]. Objetivo. O objetivo deste estudo foi descrever a epidemiologia do câncer infantil no Chile e o cenário da doença, avaliando conquistas, colaborações e futuros desafios a serem enfrentados pelo Plano Nacional de Câncer Infantojuvenil. Métodos. Este estudo descritivo apresenta uma visão geral das estratégias de colaboração nacional e inter- nacional e discute os resultados do Terceiro Relatório de Vigilância do Câncer Infantil (2017–2019), o relatório da ferramenta Pediatric Oncology Facility Integrated Local Evaluation (PrOFILE) [Avaliação Local Integrada de Estabelecimentos de Oncologia Pediátrica] do St. Jude Children's Research Hospital, a colaboração com a Organização Pan-Americana da Saúde no âmbito da Iniciativa Global para o Câncer Infantil da OMS e a elaboração do Plano Nacional de Câncer Infantojuvenil no âmbito do pacote técnico CureAll. Resultados. A análise revela o impacto do câncer infantil com base na incidência entre 2017 e 2019, englo- bando disparidades de gênero. A leucemia foi o tipo de câncer mais frequente, responsável por 40,4% dos cânceres entre crianças e adolescentes com menos de 15 anos e com uma incidência de 57,5 casos por 1 milhão de crianças nessa faixa etária. O câncer é a segunda principal causa de morte entre crianças de 5 a 14 anos. A sobrevida do câncer aumentou entre 2007 e 2019; em 2023, a sobrevida cinco anos após o diag- nóstico era 78,4%. A elaboração do Plano Nacional de Câncer Infantojuvenil envolveu a realização de uma avaliação da situação, a definição de metas e a elaboração de um plano de ação para reduzir a mortalidade por câncer na infância e aumentar as taxas de sobrevida por meio de intervenções precoces e transições descomplicadas para a atenção ao adulto. Conclusões. O Chile obteve avanços nos indicadores de câncer infantil, especialmente no aumento da sobrevida, demonstrando seu compromisso com a melhora da atenção a crianças com câncer. Isso foi alcançado por meio de marcos legais, planejamento em âmbito nacional, colaborações e participação em iniciativas mundiais. Apesar do progresso alcançado, pesquisas contínuas, implementação de políticas sóli- das e colaborações multidisciplinares continuam sendo fundamentais para enfrentar os desafios persistentes. Este estudo destaca a necessidade de aperfeiçoar os sistemas de saúde, as metodologias de coleta de dados e a cooperação mundial para assegurar a melhor atenção a todas as crianças que enfrentam câncer, melhorando assim suas chances de sobrevida e sua qualidade de vida geral.

As a part of the EIS-COVID project on the access and use of information during the COVID-19 pandemic in Chile, the objective of this paper was to ascertain how people's informational environment was constructed during the first stage of the pandemic. It discusses the results of a qualitative study of people belonging to risk groups for COVID-19: people over 18 and under 65 with chronic diseases (hypertension and diabetes) and people 65 and over. Ninety semi-structured interviews were conducted in the Metropolitan and Valparaíso regions between September 2020 and January 2021. The results reveal the problematic nature of the information overload encountered by these groups and the strategies they used to navigate it: a) information avoidance; b) content corroboration and active search for reliable sources; and c) differentiated media use.

RESUMEN Este artículo se enmarca en el proyecto EIS-COVID sobre acceso y uso de información en el contexto de la pandemia de COVID-19 en Chile. Su objetivo fue conocer cómo se constituyó el entorno informativo de las personas en la primera etapa de la pandemia. El artículo muestra los resultados de un estudio cualitativo enfocado en personas pertenecientes a grupos de riesgo por COVID-19: personas mayores de 18 y menores de 65 años con enfermedades crónicas (hipertensión y diabetes) y personas de 65 años y más. Explicar los alcances del enfoque cualitativo de investigación social en salud a estudiantes de carreras de Ciencias de la Salud resulta un desafío en la entrega de la perspectiva antropológica y social en la formación profesional. Lo anterior debido a que estos estudiantes están acostumbrados a operar en las dicotomías naturaleza/cultura y ciencia/creencia. Como docentes,implica enseñarles a observar más allá de dichas dicotomías. También exige superar la limitada comprensión que puede haber sobre lo que es la aproximación cualitativa en investigación y lo que les puede ofrecer. Por ello, el propósito del estudio es reflexionar sobre los alcances que tienen estos desafíos en el aula, recurriendo a casos de ejercicios de diseños de investigación cualitativa en la carrera de Fonoaudiología de la Facultad de Medicina de la Universidad de Chile. Concluimos que un acercamiento temprano a metodologías cualitativas permite a estudiantes de pregrado desarrollar una visión crítica hacia las dicotomías reduccionistas respecto de la producción y validación de conocimientos. Además, promueve entender la dimensión social de la salud como un fenómeno individual y colectivo. También favorece concebir a la Medicina como un campo de trabajo transdisciplinar.


To explain the scope of the qualitative approach to undergraduate students of healthcare sciences is a challenge for those of us who must translate the anthropological and social perspectives to students used to operate within nature/culture and science/belief dichotomies. This challenge involves teaching to look beyond these dichotomies and forces us, as lecturers, to overcome the barriers or limited understanding that often characterize health professionals' relation to what qualitative approach can offer to them. Based upon the teaching experience of the authors, this article aims to reflect about the emergence of these challenges, turning, therefore, to examples of qualitative research design in the speech and hearing program at the faculty of medicine of the University of Chile. We believe that an early approach to qualitative methodologies would allow undergraduate students to develop a critical vision in relation to reductive dichotomies, in addition to promoting an understanding of health as an individual and collective phenomenon, and of medicine as a trans disciplinary field of work.

BACKGROUND: Pesticides are widely used to increase crop yields and vector control. However, both acute and chronic exposure have health consequences. There is paucity of information about the global occurrence of pesticide poisonings. AIM: To characterize the reports of pesticide exposures received by a University Poison Information Center. MATERIAL AND METHODS: All pesticide exposures reported in Chile between 2006 and 2013 were analyzed. A data-collection sheet provided by the International Programme on Chemical Safety of the World Health Organization, was used to collect information. RESULTS: In the study period, 13,181 reports were analyzed. The main age groups exposed were preschoolers and adults. Sixty one percent of exposures occurred accidentally and 24.8% were suicide attempts. Exposures to acetylcholinesterase inhibitors was reported in 29.3% of cases, to superwarfarin rodenticides in 28.5% and to pyrethroids in 24.0%. An increased risk of suicide attempts with pesticides was observed among women, when compared with men (odds ratio: 1.5; 95% confidence intervals: 1.4-1.6; p < 0.001). The risk was higher among teenage girls. CONCLUSIONS: The amount of cases under acetyl cholinesterase inhibitor exposure, the most toxic pesticides currently in use should be highlighted. Workers should be educated and all cases should be reported to take actions aiming at reducing these events.

BACKGROUND: Exposure to Biological Agents during work is an emergent type of occupational risk. AIM: To characterize occupational biological risk exposure among Chilean workers which have been registered by the Toxicology Information Center, between January 2006 and December 2009. MATERIAL AND METHODS: All incoming calls reporting exposure to biological agents during the studied period were analyzed. The information obtained from the caller was registered using the Communication Record Instrument of the WHO International Programme on Chemical Safety (IPCS INTOX). RESULTS: In the studied period, 77 calls were received. The mean age of exposed patients was 35 ± 15 years and 57% of them were females. The most common involved agents were vaccines for veterinary use (42%) followed by Loxosceles laeta bites in 16%. The main routes of exposure were injections, cuts and needle stick injuries in 39% and stings and bites in 38%. The highest exposure rates were observed in Southern Chile due to self-inoculation of veterinary vaccines used in the salmon industry (22.7/100.000 actual workers). Fifty-eight percent of calls were from health care workers, and 51% of them were from health care facilities. Sixty percent of exposures occurred during summer and spring. There was a fourfold higher risk of calls involving women exposed to bites or stings (odds ratio (OR) 4.5 (CI95 1.5-13.9, p < 0.01). Men had a fourfold higher risk of being exposed to vaccines or medications for veterinary use (OR 4.2, CI95 1.4-12.6 p < 0.01). CONCLUSIONS: Most calls involving an exposure to a biological agent were caused by self-inoculation of veterinary medications.

BACKGROUND: Paracetamol (acetaminophen) is an analgesic and antipyretic drug widely used across the world. Its ingestion is one of the most common causes of drug overdose. In the United States is the first cause of acute hepatitis in adults. AIM: To describe the epidemiological profile of paracetamol overdose in Chile. MATERIAL AND METHODS: Cross sectional retrospective study that included all the phone call inquiries received at the Poison Control Center of the Pontificia Universidad Catolica de Chile (CITUC) during 2009. RESULTS: Nine hundred fifty nine inquiries involving acute paracetamol exposures were received. Women represented a 63.1% of the cases. Half of the cases were suicide attempts, of which 74.8% were women. Accidental exposures occurred mainly in children. In 29.3% of the patients, the exposure was considered to involve a hepatotoxic dose. Women had 2.7 times the risk of men to ingest a toxic dose of paracetamol with suicidal purpose (Odds ratio (OR) = 2.7; 95% confidence interval (Cl): 2.1-3.6; p < 0.001). Adolescents had 3.4 times the risk of the general consultants (OR: 3.4; 95% Cl: 2.4-4.7; p < 0.001). CONCLUSIONS: Paracetamol overdose is common in Chile. Authorities should carry out preventive measures. Antidotes and the capacity to measure plasma levels of paracetamol should be available in healthcare centers.

Background: Paracetamol (acetaminophen) is an analgesic and antipyretic drug widely used across the world. Its ingestion is one of the most common causes of drug overdose. In the United States is the first cause of acute hepatitis in adults. Aim: To describe the epidemiological profile of paracetamol overdose in Chile. Material and Methods: Cross sectional retrospective study that included all the phone call inquiries received at the Poison Control Center of the Pontificia Universidad Catolica de Chile (CITUC) during 2009. Results: Nine hundred fifty nine inquiries involving acute paracetamol exposures were received. Women represented a 63.1% of the cases. Half of the cases were suicide attempts, of which 74.8% were women. Accidental exposures occurred mainly in children. In 29.3% of the patients, the exposure was considered to involve a hepatotoxic dose. Women had 2.7 times the risk of men to ingest a toxic dose of paracetamol with suicidal purpose (Odds ratio (OR) = 2.7; 95% confidence interval (Cl): 2.1-3.6; p < 0.001). Adolescents had 3.4 times the risk of the general consultants (OR: 3.4; 95% Cl: 2.4-4.7; p < 0.001). Conclusions: Paracetamol overdose is common in Chile. Authorities should carry out preventive measures. Antidotes and the capacity to measure plasma levels of paracetamol should be available in healthcare centers.

Objetivo: Describir y caracterizar la epidemiología de las consultas por intoxicaciones al Centro de Información Toxicológica de la Universidad Católica (CITUC) y conocer las variaciones epidemiológicas producidas entre los años 2004 y 2007. Método: Se realizó un estudio retrospectivo, observacional y descriptivo de las llamadas registradas por el Centro durante el periodo del año 2007. Se analizaron las siguientes variables: número total de llamadas, lugar de las llamadas, circunstancias de la exposición, tipo y números de agentes involucrados, vías de exposición, tiempo desde la exposición, características demográficas y las indicaciones y tratamientos recomendados. Resultados: Se registraron 30.171 llamadas. El 45 por ciento de las consultas provenían del hogar y el 52,5 por ciento de las instalaciones de salud. El 67,2 por ciento de las consultas fueron exposiciones accidentales y el 29,2 por ciento fueron intencionales. Los Medicamentos se presentaron en un 57,1 por ciento de las llamadas, seguidos por los Productos de uso doméstico (16,5 por ciento), Productos de uso industrial (4,7 por ciento), y Pesticidas (5,8 por ciento). La ingestión fue la vía de exposición que se predominó en un 76,6 por ciento del total de casos. El 23,3 por ciento del total de las llamadas, se realizaron dentro de la primera hora post-exposición. Las intoxicaciones en niños menores de 14 años correspondieron al 50 por ciento de las llamadas. Respecto al año 2004 aumentó la proporción de llamadas procedentes del hogar y el porcentaje de intoxicaciones intencionales. Conclusiones: Los medicamentos constituyen el principal agente de intoxicación. La principal vía de exposición es la ingesta y el principal grupo de población afectado son los niños menores de 14 años. Objective: To describe the epidemiological features of the intoxication cases reported to CITUC and the variations produced between 2004 and 2007. Method: A retrospective and descriptive study of the calls registered at the Center during 2007. The following data were analyzed: number and place of the calls; circunstances of exposure; type and number of agents involved; routes of exposure; time since exposure; sex and age of the subjects, and treatment and advice offered. Findings: 30.171 calls were received, 45 percent from the home and 52,5 percent from Health Care Service; 67,2 percent were accidents and 29,2 percent were intentional exposures. The agents were: medicines (57,1 percent); cleaning products (16,5 percent) pesticides (5,8 percent); other chemical and industrial products (4,7 percent). The exposure route was oral in 76,6 percent of cases; 48 percent of calls occurred within one hour of exposure. Children under six were the main group involved. Conclusion: The evidence substantiates a need for prevention campaigns, sponsored by the Health Ministry, about the correct storage of medicines and the appropiate management of poisoning at home. Child-resistant containers and education are two essential elements for prevention of pediatric intoxications.

Resultados: Se registraron 22.168 llamadas. El 52,5 por ciento de las consultas provenían del hogar y el 40,2 por ciento de los Servicios de Urgencia. El 71,6 por ciento de las consultas eran exposiciones accidentales y el 19,8 por ciento fueron intencionales. Los medicamentos se presentaron en un 53,6 por ciento de las llamadas, seguidos por los Productos de Aseo (13,7 por ciento), Productos industriales y Químicos (10,2 por ciento), y Pesticidas (7,1 por ciento). El 92,5 por ciento de las exposiciones eran con un agente tóxico. La ingestión fue la vía de exposición que se registró en un 82 por ciento. El 44,6 por ciento del total de las llamadas, se realizaron dentro de la primera hora post-exposición. Las intoxicaciones en niños menores de 6 años correspondieron al 47,9 por ciento de las llamadas. Conclusiones: Los medicamentos constituyen el principal agente de intoxicación. La principal vía de exposición es la ingesta y que el principal grupo de población afectado son los niños menores de 6 años. Estos son indicadores evidentes y claros de la necesidad imperante de desarrollar campañas publicitarias preventivas e informativas, avaladas por el Ministerio de Salud, respecto al correcto almacenaje de los medicamentos y el manejo de las intoxicaciones en el hogar. La introducción del envase resistente a niños y la educación constituyen dos piezas fundamentales en la prevención de las intoxicaciones pediátricas.

BACKGROUND: The existence of Poison Centers for management and prevention of intoxications has been endorsed by the international experience. In Chile, the Toxicological Information Center at the Pontifical Catholic University of Chile has been active since 1992, receiving about 130,000 calls until 2002. AIM: To analyze the statistical data gathered throughout the first ten years of our Research Center. To delineate the epidemiological pattern of intoxications in Chile. MATERIAL AND METHODS: Retrospective study in which records from calls for toxicological information received during the 1992-2002 period reviewed. Analyzed data were total calls per year, place of call, exposure circumstances, age, sex, route of exposure and involved agents. RESULTS: 96,468 calls analyzed. The main exposure circumstance was "unintentional" (78.6%), followed by "intentional" (16.9%). Intoxications in children under 5 years old motivated 50% of calls. According to route of exposure, ingestions involved 75,992 calls (78.8%). Medications were the most common substances, accounting for 49.2% of calls, followed by cleaning products (12.1%), pesticides (11.3%), industrial and chemical products (10.5%) and cosmetics (2.7%). Medications acting on the CNS were the most recurrent, with 19,096 reports. CONCLUSIONS: The epidemiological pattern for intoxications in Chile is very similar to that reported in developed and other Latin American countries. Children under 5 years old, are a high risk group for intoxications. It is imperative to improve the recording and follow-up of patients that call to the Center, to improve epidemiological data of intoxications in Chile.
